All guests attending the Meeting in person are asked to take a rapid antigen Covid test (an “at home” test is perfectly fine) before arriving at the meeting (Wednesday, November 16-18), as close to the start of the meeting as possible. We have a limited supply of COVID tests that we can provide you upon request. You will need to attest to testing negative prior to entering the meeting by submitting the health screening form below.
Upon completing the questionnaire, we will send you a confirmation email that the completed form has been received and then a second email showing that you are cleared to enter the premises. Please save this email as you may be asked to show it upon your initial arrival at the Harborview Ballroom, at the Battery Wharf Hotel. Once cleared for initial entrance, subsequent testing/attestation will not be required.
